I hereby apply to become a Patron of the Southend-on-Sea Operatic & Dramatic Society subject to the rules of that organisation and, within one month of acceptance, I agree to pay the current subscription.

The annual subscription fee agreed by the AGM of 25th April 2009 is £25 (twenty-five pounds).
